In intimate scenes, how the hero addresses the heroine changes the temperature of the scene.
Place them in close quarters. Hostel, office, bus stop, or neighboring houses. Physical proximity is necessary. The Akka should be doing something domestic (washing clothes, cooking, praying) while the Thambi watches her.
The "Thambi" has loved his "Akka" since childhood. She marries someone else. The marriage fails (due to abuse or neglect). The "Thambi" rescues her. This is the most "dark" and dramatic version of the genre, often dealing with trauma and healing. To give you a flavor of the genre,
The most common setting. The heroine loses her parents; the hero’s family adopts her. She is "Akka" legally but not by blood. Years later, the "Thambi" returns from abroad a changed man. He doesn’t see an "Akka"; he sees the woman he wants to marry. The family’s reaction forms the climax.
Here, the "Akka" is a senior in engineering college, mentoring a junior "Thambi." The "Akka" has a boyfriend (who is usually a villain). The "Thambi" silently protects her. When the boyfriend betrays her, the "Thambi" steps in, revealing his love. The conflict here is social status and age gap (usually 2-4 years).
